FOX Corporation will ensure the safety of its personnel for routine, non-routine, or special work activities through the performance of job safety analyses prior to performance of the work.
1.0 SCOPE
This Procedure describes the process for analyzing and controlling hazards associated with routine, non-routine, or special work activities, and applies to
all such work activities performed by FOX Corporation.
2.0 Definitions
Job Safety Analysis (JSA): a pre-planned evaluation of the component parts of any method or procedure for the following purposes:
1. To determine the hazards connected with the work and the requirements or qualifications of those who are to perform the work
2. To identify hazard control methods that can feasibly be applied
3. To implement solutions to eliminate, or reduce to a minimum, the potential consequences of identified hazards.
3.0 Responsibilities
In addition to required Environmental, Health and Safety (EHS) Program responsibilities discussed in EHS001,
EHS Policy, the responsibilities described below are necessary to ensure the effectiveness of the procedure.
3.1 Department Heads
Department Heads will:
1. Ensure that their employees are aware of their responsibilities for evaluating hazards of new, non-routine, or special work activities.
2. Ensure that their employees participate in the training and development of JSAs during their work activities.
- Ensure that Job Safety Analysis forms (Attachment 1) are completed for all such activities.
3.2 Environmental, Health and Safety (EHS) Department
The EHS Department will:
- Evaluate and approve all Job Safety Analysis forms (Attachment 1)
- Ensure that assistance is provided to Departments responsible for developing JSAs.
4.0 Procedure
1. JSAs must be prepared for routine, non-routine, or special work activities for which the process, equipment, or procedures indicate potential for injury
and/or property damage exists.
2. Activities performed in accordance with an Activity-Specific EHS Procedure must satisfy the requirement for a JSA through correct application of the
specific procedure.
4.1 Completion of the JSA Form
1. The Job Safety Analysis form (Attachment 1) will:
a. Identify the JSA author
b. List the key activities in the sequence in which they occur
c. Determine the hazard(s) and/or hazardous material(s) involved for each step
d. Be specific in designation of required protective devices, equipment, or clothing
e. Apply specific, effective safety measures to eliminate or control the hazard(s).
2. JSAs must be developed in sufficient detail and supplemented by the Job Sequence form (Attachment 2) to prevent confusion and misunderstanding
by later reviewers, or by the personnel examining the JSA before performing the work.
3. JSAs must be submitted to the EHS Department for review and acceptance/signature.
4.2 Worker Sign-Off
All personnel involved in work activities for which JSAs are prepared, are responsible for signing the Job Safety Analysis Sign-off form
(Attachment 3), indicating that they reviewed, understand, and will comply with the provisions of the JSA.
Department Heads must ensure that the JSA is maintained in a conspicuous location for the duration of the work.
4.3 Posting And Filing of JSAs
- A completed copy of the JSA must be available for review by the employees.
- The Department managing the work must retain a copy of the JSA.
