FOX Corporation, state and federal occupational safety and health regulations require that any on-the-job injury, illness, or accident will be promptly reported. All injuries, accidents, or near misses will be investigated to enable identification of causal factors and prevention of future occurrence.
1.0 SCOPE
This procedure applies to all FOX Corporation (FOX) activities when an injury, accident, or near miss occurs.
2.0 Definitions
Incident:
an injury, illness, or accident involving an employee or member of the public, regardless of the extent of damage. Near Miss incidents are includes as
defined below.
Near Miss:
any non-injury safety incident, when an injury to an employee or member of the public could have occurred, or when more than $1,000 in property damage
occurs.
Serious Injury:
an incident involving serious injury or illness as defined by any one or combination of the following:
· Requires 24-hours or more of inpatient hospitalization for treatment (not just observation)
· Results in loss of any member of the body
· Produces serious permanent disfigurement
· Causes death.
3.0 Responsibilities
In addition to required Environmental, Health and Safety (EHS) Program responsibilities discussed in EHS001, EHS Policy, the Incident Reporting and
Investigation responsibilities described below are necessary to ensure the effectiveness of the procedure.
3.1 Division Heads
Division Heads will:
- Ensure that all division-level personnel are aware of the incident reporting requirements in this Procedure.
- Support and participate as a member of the Incident Review Committee (see Section 4.2) for all incident investigations of serious injuries.
- Ensure that adequate funds are allocated at the division or department-level to ensure effective investigations of near misses, incidents, and serious
injuries.
3.2 Department Heads
Department Heads will:
- Ensure that all department-level personnel are aware of the incident reporting requirements in this Procedure.
- Ensure assigned managers and supervisors effectively report, investigate, and document all incidents.
- Support and participate as a member of the Incident Review Committee (see Section 4.2) for all incident investigations of serious injuries.
3.3 Supervisors
All supervisors will:
- Be aware of their requirements for incident reporting under this Procedure.
- Immediately report incidents to the EHS Department and refer the injured employee to a medical provider for treatment and evaluation.
- Conduct and document investigations of all incidents and near misses that involve assigned employees.
- Participate as a member of the Incident Review Committee (see Section 4.2) for all incident investigations of serious injuries.
3.4 Employees
All employees will:
- Be aware of their requirements for incident reporting under this Procedure.
- Immediately notify their supervisor of any incident or near miss that occurs on the job.
- Participate as required in any investigation conducted in response to a workplace incident.
3.5 Environmental, Health and Safety Department
The EHS Department will:
- Maintain an on-call EHS representative who can be reached at any time, regarding an accident or injury.
- Notify OSHA regarding serious workplace injury events.
- Maintain the OSHA Form 300 – Log of Work-Related Injuries and Illnesses and supporting forms for FOX.
- Review the investigation documentation for all incidents and assist in the identification of appropriate corrective actions to prevent similar
incidents in the future. - Conduct the accident investigation for all serious injuries.
- Perform a periodic review of this Procedure (at least annually) to ensure that all requirements are current and meet regulatory requirements.
4.0 Procedure
4.1 Incident Reporting Procedure
4.1.1 Medical-related Incidents
Any on-the-job incident must be promptly reported in accordance with the following procedure:
- The supervisor of an injured employee must be notified immediately.
- Dial 9-1-1 (9-9-1-1 on the Fox Lot) for any medical emergency involving loss of consciousness or other serious condition.
- For non-emergency medical-related incidents, the employee’s supervisor must ensure that the injured employee receives medical attention as soon as
possible. Specific procedures for each operating location should be developed as appropriate. - For medical-related incidents that occur on the Fox Studios Lot during normal business hours, the employee must report to the Medical Department,
located in Building 99, Room 220, immediately for evaluation. The Medical Department will determine if the employee requires off-site medical care. - Supervisors are required to notify the EHS Department of the incident as soon as possible by calling 310-369-2671 or the Employee Hotline at
866-369-2225, Option 9 and/or visiting www.foxsafety.com to submit the information online. During non-operating
hours, the on-call EHS representative can be reached by calling the Security Department at 310-369-6555 and requesting a safety representative be
contacted.http://www.fngsafety.com/ - The EHS Department notify the applicable Human Resources, Workers’ Compensation
4.1.2 Non-Medical-Related Incidents and Near Misses
- Supervisors must be notified immediately.
- Supervisors are required to notify the EHS Department of the incident/near miss as soon as possible by calling 310-369-2671 or the Employee Hotline at
866-369-2225, Option 9 and/or visiting www.foxsafety.com to submit the information online. - The supervisor and affected employee are required review the recommendations of corrective action(s) with a focus on prevention of recurrence.
4.1.3 Follow-Up Actions
- Each employee involved in an occupational injury or illness will be provided with a copy of an appropriate Workers’ Compensation Forms. Filing of this
form is the responsibility of the employee and is required to initiate a Workers’ Compensation Claim. - The EHS Department will evaluate each incident to determine if it qualifies as an “OSHA Recordable” incident.
- Qualifying incidents will be entered onto FOX’s OSHA Form 300 and maintained by the EHS Department.
- Incident investigations will be performed in accordance with Section 4.2, below.
- The EHS Department will:
- Determine if an incident qualifies as a “serious workplace injury event”
- Be responsible for providing any required notification to OSHA.
4.2 Investigation Procedure
Incidents and near misses require investigation to identify factors that might have caused or contributed to the occurrence and to prevent future
incidents.
The investigation procedures and level of details obtained will be determined by the severity of the incident.
4.2.1 Incidents
Incidents are more severe than near misses and can result in significant injury or property damage.
Department Heads will conduct investigation in accordance with the following:
- Photographs of the incident scene must be taken as soon as possible after the incident.
- Statements from involved employees and witnesses must be collected.
- The Department Head will document the investigation.
- The investigation must be conducted within four days after the incident occurs with the completed Workers’ Compensation Form(s), all applicable witness statements and other information.
5. The completed investigation package and recommended corrective actions will be reviewed by the EHS Department. As necessary, the EHS Department may
require Department Heads to obtain additional information surrounding the incident in order to fully satisfy the investigation requirements/needs of the
incident.
4.2.2 Near Misses
Near miss incidents are the least severe, but can serve to identify and correct causal factors, which might otherwise contribute to more severe future
incidents.
Supervisors for the group incurring the incident will document the investigation with assistance from the EHS Department. The Supervisor must complete the investigation within four days after the incident occurs. Any recommended corrective actions and other information must be submitted to the EHS Department for review and recordkeeping.
4.2.3 Serious Injuries
The investigation of serious injuries will be conducted by the EHS Department and depending on type or severity, may require participation or oversight by Human Resources or Legal. Department Heads will ensure that applicable personnel are available and cooperate with the investigation process.
