FOX Corporation will provide its employees, freelance daily personnel and visitors with a clear plan for evacuation of the facility in the event of an emergency, and ensure that all employees, visitors, and other personnel will be evacuated safely.
1.0 SCOPE
This Procedure addresses evacuations for FOX Fox Lot work areas and facilities for the protection of employees and others from emergencies that could threaten their safety and health.
2.0 Responsibilities
2.1 Fox Security
Fox Security will:
- Respond to emergencies to provide control of vehicles, personnel egress, and in some cases, facilitate building evacuations (Bldg. 101).
- Coordinate with the officer- in-charge at the scene and local emergency service personnel as needed.
- Prevent unauthorized entry or re-entry into buildings and reduce loss or theft of company and private property.
2.2 Floor Wardens and Applicable Supervisory Personnel
There are floor wardens assigned to Buildings 100 and 103. The Floor Wardens and in some cases, Supervisors must:
- Ensure that personnel move to a safe area after a building alarm has been sounded or other manually activated method of evacuation notice has been used.
- With the assistance of Security personnel, ensure the safe flow of personnel along approved evacuation routes.
- Assist persons with disabilities in accordance with Section 3.5 of this procedure or other pre-planned procedures as needed.
- Recognize the presence of potential evacuation-related hazards and know applicable evacuation procedures in order ensure proper evacuation of personnel.
- Report discovery of any employees or visitors missing to the emergency services personnel.
- Provide appropriate instructions to affected employees and visitors when an emergency no longer exists and it is safe for personnel to return to vacated buildings or facilities.
2.3 Designated Floor Warden Alternates
Floor Warden Alternates must be familiar with and be capable of performing all responsibilities assigned to Floor Wardens.
2.4 Supervisors
Supervisors will:
- Ensure assigned employees are aware of established evacuation procedures and evacuation routes in proximity of their employees, and review any suggested changes in either the employee’s responsibilities or changes in the Plan.
- Ensure Floor Wardens (Primary and/or Alternate) are selected if they manage employees assigned to physical offices or workstations designated as Floor Wardens locations.
- Prepare a specific written procedure for any critical operation that would delay evacuation.
- When an evacuation occurs, ensure that all assigned employees are accounted for.
2.5 Employees
Employees will know the applicable Safe Refuge Area identified for their assigned building/work location to ensure their individual safety in the event of an emergency. Employees will:
- Know the location of fire alarm pull boxes.
- Be familiar with secondary exit route in case primary exit route is blocked.
- Stop work and immediately evacuate the building when the evacuation alarm sounds, and assist anyone along the approved evacuation routes to a safe area.
- Report to the Floor Warden once outside the building in designated area until told to report to a shelter area or that the building is safe to re-enter by Name or Name.
- Post evacuation routes and Safe Refuge Area Maps in a location in office or work area and indicate present location on the building layout.
2.6 Environmental, Health and Safety Department
The EHS Department will perform a periodic review of this Procedure (at minimum, annually) to ensure that all procedures are current and meet established regulatory requirements.
3.0 Procedure
3.1 Safe Refuge Areas
Applicable Safe Refuge Areas are presented in Attachment A, Safe Refuge Area Maps.
3.2 Evacuations
3.3 Evacuation Procedures
- All building occupants must evacuate the building promptly, using the closest building exit.
- Occupants of the building must proceed to the designated SRAs (See Attachment A). Visitors should accompany the employee they are visiting to the designated assembly area.
- All building occupants should report directly to the applicable Floor Warden that will be positioned in their applicable safe refuge area to confirm that they have left the building.
- If employees are able to do so, they should assist individuals with physical disabilities out of the building and to the assembly area. If they do not feel able, they should seek assistance from other employees, Floor Wardens or Emergency Services personnel.
- During an evacuation all employees without an assigned emergency role will leave the building promptly. They will use the emergency exit closest to their location at the time of the evacuation alarms(s), announcement or the activation of the sprinkler system.
- Employees shall NOT return to their work area to retrieve personal belongings. Once outside the building, employees shall NOT be allowed to leave the property, or to go to their vehicles, until authorized via all clear communications by Emergency Services personnel.
- During an evacuation, all visitors shall exit the building using the nearest emergency exit to their location at the time of the evacuation announcement.
3.4 Accounting for Visitors
Visitors will be accounted for following an evacuation and may need additional assistance when exiting. All visitors and contractors will sign in when entering the workplace; the list will be used when accounting for all persons in the assembly area. The hosts of visitors, Floor Wardens, and designated assistants in the area where visitors are located at the time of an evacuation will help visitors safely evacuate.
3.5 Persons with Disabilities
3.5.1 Assistance
- In most cases, evacuation will not be necessary or advisable. Do not evacuate unless instructed to do so by emergency personnel, the fire alarm is activated or danger is imminent.
- All persons should proceed toward the nearest safe exit as instructed by emergency personnel. When a person with a disability reaches an obstruction, such as a staircase, he/she should request assistance from others in the area.
- If assistance is not immediately available, stay in the exit corridor and call for help. If the exit corridor should become dangerous (smoke, fire), proceed into the stairway, if possible, and stay at the stairway landing. Close the door behind you to keep smoke out of the stairway. If the stairway should become unsafe, proceed to a safe area away from smoke and fire, closing doors behind you to isolate the smoke. Call 9-9-1-1 and advise the operator of your situation. If in a room with a window, signal rescuers by waving or placing a sign in the window. Do not open the window unless smoke is entering the room. If possible, place a wet cloth material around and under the door to prevent smoke from entering.
- Persons who cannot speak loudly should carry a whistle or other means of attracting assistance. NOTE: It is suggested that individuals with mobility, visual or hearing disabilities prepare for an emergency ahead of time by informing their building coordinators, floor wardens, co-workers and classmates of the best methods of assistance during an emergency.
3.5.2 Persons Who Cannot Walk (Non-Ambulatory)
Always consult the person about the following:
- Preferred ways of being removed from wheelchair. Wheelchairs should not be used in stairwells, if at all possible.
- Whether to extend or move extremities when lifting because of pain, catheter leg bags, spasticity, braces, etc.
- Whether a seat cushion or pad should be brought along with the person if they is removed from the chair.
- Being carried forward or backward on a flight of stairs.
- After-care if removed from the wheelchair (i.e. whether the person prefers a stretcher, chair with pad or medical assistance).
3.5.3 Person with Visual Disabilities
- Tell the person the nature of the emergency. Offer to guide them to safety.
- As you walk, say where you are and advise of any obstacles.
- When safety is reached, help to orient the person and ask if additional assistance is needed. Do not leave them alone.
3.5.4 Person with Hearing Disabilities (persons who may not hear audible alarms and are in buildings that do not have visual alarm devices)
- Write a note explaining the nature of the emergency. Include “Go to your nearest emergency exit – NOW” OR
- Turn light switch on and off to gain attention, then indicate through gestures or writing what is happening and what to do.
3.6 Discovery of Emergency
The types of emergencies to be reported by facility personnel are:
- Medical
- Fire
- Earthquake
- Severe Weather
- Bomb threat
- Chemical spill
- Extended power loss
- Other, e.g., Active Shooter, Terrorist Attack, Hostage Taking.
Any person who discovers a fire or other emergency will follow these reporting and escape procedures:
- When a fire is seen, or another emergency condition becomes known, walk to a safe area and warn others along the way.
- Immediately Activate the Nearest Emergency Alarm Pull Box/Dial 911/Dial 9-911/Dial Extention xxxx/or Other Contact. The caller will state his or her name, location, type of help needed, and stay on the telephone until released by the dispatcher.
- After activating the alarm, proceed to the emergency assembly area.
3.6.1 Fire or Explosion Emergency
If an employee discovers a fire or sees an explosion:
- Immediately sound the fire alarm by activating nearest fire alarm pull station [describe the appropriate procedure if different].
- If trained and authorized to extinguished incipient fires, attempt to extinguish the incipient fire if the employee determines it is safe to do so.
- If unable or untrained to extinguish the fire, close–but DO NOT lock–the door and go to the evacuation assembly area.
All other employees will follow the general escape procedures for all emergencies. If the primary escape route is blocked, proceed to the secondary escape route. If all escape routes are blocked with smoke, fire, or impassible objects:
- Shelter in place in an enclosed room or other area that is free of smoke and fire, if possible.
- Shut–but do not lock–doors behind you. This will help smother the fire, or block the flow of fumes, gases, or smoke.
- Stay low to the ground if near smoke and fire.
- Use any means possible to get the attention of rescuers, including calling 911 on fixed or cell phones if functioning, banging on walls, yelling, and other loud noises.
- Do not move from the shelter area until directed by rescuers.
3.6.2 Power Outage
In the event of a power outage, a Floor Warden or supervisors will instruct building occupants to take the correct course of action depending upon the anticipated length of the outage.
During electrical outages due to severe weather, building occupants will be requested to stay in the building until the weather improves. If the weather is not severe, Name may make the decision to close the building if it appears to be a prolonged outage.
If personnel are in an unlighted area at the time of the outage, they will proceed cautiously to an area that has emergency lights if so directed by a Floor Warden or supervisor. Otherwise, personnel will remain in place until help arrives.
Do not attempt to use elevators during a power outage. If a power outage occurs while an elevator is occupied, do not attempt escape if the elevator car stops between floors or doors do not open. [Options: Use the emergency telephone in the elevator car to call for help/Use a cellphone to call 911/Use a cellphone to call the emergency number posted in the elevator]. Rescue will be performed by Department/Emergency Service.
3.6.3 Hazardous Chemical Spill
The following are the locations of information and equipment for responding to a chemical spill:
- Spill containment and security equipment: Location(s)
- Personal protective equipment (PPE): Location(s)
- Material safety data sheets (MSDSs): Physical Location of File System or Electronic Access
Small Spill
Evacuate all personnel not trained in emergency spill response from the immediate work area where the spill is contained. Do not permit anyone to enter the spill area, touch or otherwise contact the spilled material, or place themselves at risk unless they have appropriate training and personal protective equipment.
Large Spill or Gas Emissions
If the spill emits toxic or noxious gases, or if the spill is more than Designate a Minimum Volume evacuate the entire facility or building. Make sure that assembly areas are relocated or moved upwind of the spill. Name will contact outside agencies including any of the fire, police, emergency medical, health, or emergency management departments if the chemical spill is large. Name will also contact the federal and state spill hotlines if at any time during the response Name determines that the spill or release represents a hazard to the environment or community.
3.6.4 Bomb Threat
All employees will be evacuated from the facility in the event of a bomb threat. Following are the general procedures for handling a bomb threat announced over the phone:
- Notify Name or Department at Phone Number/Extension.
- Immediately after above notification, complete the Bomb Threat Report as completely and as accurately as possible. See Attachment Number for a copy of the Bomb Threat Report.
- Notify your immediate supervisor that you have received a threatening phone call and have already notified Name or Department.
- After completion of the Bomb Threat Report, remain at your location until Name arrives. They will interview you regarding the call and take your completed Bomb Threat Report.
3.6.5 Severe Weather and Natural Disasters
In the event of severe weather or other natural disaster, all employees will be instructed to follow the specific procedures for each type of natural event.
3.6.6 Shelter-in-Place
- In the event of an off-site hazardous chemical release or other event that makes an evacuation of the facility dangerous or impossible, employees will take shelter in place until it is safe to evacuate.
- Floor Wardens and Designated Assistants will perform the duties for shelter-in-place procedures.
- Employees, visitors, and other personnel will be notified to take shelter in place by Means of Notification. Shelter-in-place areas are Location(s), Room Number(s).
3.6.7 Critical Operations
Plant and Technical Operations personnel will identify any critical operations or processes that must be shut down or inactivated before an evacuation is completed, and will designate the operations and the personnel who will implement the shutdown or inactivation. During some emergency situations, it may be necessary for some specially assigned personnel to remain at the work areas to perform critical operations.
Personnel involved in critical operations may remain on the site by permission of the site- designated official or Name. In case the emergency situation will not permit any personnel to remain at the facility, the designated official or other assigned personnel will notify Name to initiate backups.
4.0 Training
The EHS Department will train all Floor Wardens and Alternates to assist in the safe emergency evacuation and to conduct a thorough post-evacuation accountability of all personnel. Monitors will be trained in the complete workplace layout and various alternative escape routes if the primary evacuation route becomes blocked. Floor Wardens and Alternates will be made aware of employees with special needs (who may require extra assistance during an evacuation), how to use the buddy system, and of any hazardous areas to avoid during an emergency evacuation.
If necessary, Plant Operations personnel will train employees designated to perform emergency shutdowns how to recognize when to abandon the operation or task and evacuate themselves before their exit route is blocked.
5.0 Evacuation Drills
All employees will participate in evacuation drills as follows:
- When the fire alarm signal sounds, promptly leave work areas and proceed to the nearest exit.
- Elevators will not be used, except under the direction of the fire department.
- Proceed to the designated assembly area. Do not go to cars or otherwise leave the assembly area until notified to do so by proper authorities.
- After the drill, employees will be reminded to know their alternate exits.
- A record of evacuation training for employees will be maintained for Duration at Location.
