CSG01 - Contractor EHS Policy Acknowledgement Form

The commitment to EHS is a key objective and extends to all levels of the organization. FOX requires all work operations to be conducted safely and in accordance with established EHS procedures and applicable regulatory requirements.

By acknowledging our requirements by checking off each box, Contractors employed by FOX Corporation (FOX) who work at any event for the Marketing or Creative Services Groups, confirm adherence to all FOX environmental, health and safety (EHS) procedures, as well as any applicable Federal, State, Regional and/or Local regulations. This form outlines the roles and responsibilities of Contractors in providing a safe work environment for its employees, subcontractors and anyone that may be affected by Contractors at the event.
General Safety Requirements
- Contractor is responsible for awareness and full compliance with all applicable rules, regulations, laws, and practices applicable to their event and their subcontractor's work (if applicable).
- Contractor is expected to perform its operations in accordance with its proper and appropriate safety policies and procedures, to ensure that hazards associated with the performance of the event activities are identified, controlled and mitigated in a timely fashion.
- Contractor shall exercise the utmost care to protect its personnel, subcontractors and any other employees at the worksite by providing safe working conditions. Extensive care should be taken at all times to avoid environmental damage as well.
- Contractor shall provide all necessary safety training, safety equipment, and personal protective equipment (PPE) to its personnel and subcontractors prior to working.
- Contractor is required to conduct routine safety meetings informing employees of any relevant safety policies and procedures for the specific work to be done. Contractors must provide documentation of safety meetings to any FOX personnel upon request.
- Upon request, Contractor shall provide proof of an implemented health and safety program to any FOX representative. Components of the health and safety plan must include roles and responsibilities of Contractor personnel, designated person at the event who will be responsible for safety and who will coordinate the efforts and provide general facilitation of the Contractor’s safety program, system to identify, signal, communicate, and correct hazards and means to ensure compliance with all general and activity-specific training requirements, including the proper use of PPE that will be used at the worksite.
- Contractor acknowledges that the FOX EHS Department develops an event-specific HASP and/or EAP. It is the responsibility of the Contractor to review the plan(s) in their entirety and brief its employees and subcontractors based on the information within the HASP and EAP.
Roles and Responsibilities
Contractor
- Contractor understands that it must promptly correct any hazards that its employees, or subcontractors, create. It is also the responsibility of the Contractor to notify FOX personnel, other contractors, bystanders and all affected parties about any hazards created. Contractor must escalate any EHS concerns to the FOX Event Coordinator. The FOX Event Coordinator will work with the EHS Department and the Contractor (Creating Employer) to correct the hazard in a timely fashion.
Supervisors
- Contractor personnel with supervisory power must communicate EHS responsibilities to their assigned team members. They must ensure that team members complete required training and participate in the overall safety of the event. Supervisors must also assess their work areas, investigate all accidents and incidents and correct recognized hazards. Supervisors must notify the EHS department of any incidents by calling 310-369-2671 or the Employee Hotline at 866-369-2225, option 9.
Employees
- All Contractor employees must play an active role in ensuring the safety of the event or location. This includes, but is not limited to following safe work practices, completing required EHS training, reporting accidents, incidents or near misses, attending safety meetings and informing supervisors of hazards they find at the event or location.
Training
General and Activity-Specific
- Contractor will ensure that its employees and subcontractors receive employee safety training which includes general safe work practices as well as instruction on control of hazards unique to the event. Contractor will ensure that its employees and subcontractors maintain applicable safety training credentials at the intervals required by Federal, State, Regional and/or Local requirements.
Powered Industrial Trucks
- All personnel who operate powered equipment must be trained and certified as per Federal, State, Regional and/or Local requirements. This equipment includes, but is not limited to: forklifts, scissor lifts, boom lifts, telehandlers, etc. This equipment will be operated, inspected and maintained as per relevant regulations.
Incident Reporting, Investigation and Inspections
Incident Reporting
- Contractor will notify FOX of accidents or incidents involving contractor personnel and/or equipment that occur at its event locations while performing services for FOX.
Incident Response
- In the event of a medical emergency, if able, the employee involved must call the Employee Hotline at 888-369-2225, Option 9. A supervisor, medic or security representative should make the call if the employee involved is unable to do so. If there is a medical emergency involving loss of consciousness or other serious condition, dial 9-1-1 immediately (or local emergency services) immediately.
Incident Investigation Process
- Contractor shall follow the incident reporting and investigation process established by FOX. This requires any on-the-job injury, illness, accident, or near-miss to be promptly reported to a supervisor. Notices, or any other type of violations, from a regulatory agency will also be brought to the attention of FOX and must be promptly addressed and corrected.
- The EHS Department investigates all incidents to determine the root cause, and prescribe any corrective or preventive action necessary to prevent the incident from occurring in the future. Contractor will provide full cooperation during the investigation process.
Inspections
- The EHS department has the right to walk through and inspect any event, location or worksite under FOX operations in order to ensure EHS compliance. If it is observed that work conditions are not safely managed or maintained, FOX personnel have the authority to issue a Stop Work Order to cease all work, until the operation is deemed safe to return
